I hereby grant to the Artist the irrevocable right to the use of my likeness in any and all media and in any and all manners including composite or segmented representation for the preparation of works of art or any other lawful purpose. I hereby waive my right to approval/rejection of the completed works, which may incorporate my image, and hold harmless the Artist and the Artist's Representatives against any and all liability arising herein. I understand that any and all photos submitted to, or obtained by the Artist via a photo shoot, become property of the Artist.
	grant the Artist permission to also use my name for the title and/or description of artwork in which my likeness is featured/included.
By electronically submitting a photo of myself, I agree to the terms and conditions of this Model Release Form.
